Instructions: To request a fabric sample, please enter your name, contact info, shipping address and the fabric model number in the form below. Make sure you enter your mailing/physical address correctly! If the address is not entered correctly, your sample packages may be returned as undeliverable. You would then have to place a new order. Save time and avoid frustration by entering the address information in the appropriate boxes and double-checking for typos and other errors. You may select up to 3 samples at a time. Thank you.
